Sorry to be a stickler... but you didn't answer the question in the title. The answer is three.

"Three cosmonauts also died in a depressurization accident in 1971. This incident is the only one to actually occur outside Earth's atmosphere, Hermanson said."

All of the rest of these accidents happened within the Earth's atmosphere and therefore not in the accepted definition of "space" and would only qualify as space-related accidents.

Apollo 1 was not “bound for space” on that day. It was a pre launch test only. Of course that does not diminish the significance of the tragedy.
